Science Matters When it Comes to Fluoride Varnish

Preventive dentistry continues to evolve as new science helps clinicians better understand how fluoride varnishes work and what factors influence their effectiveness. In the recent Dentaltown article “What We Know About Fluoride-Releasing Varnish,” scientist and fluoride expert Clifton Carey, PhD explores the chemistry behind fluoride varnishes and explains why adhesion and contact time play such a critical role in long-term enamel protection. The article provides valuable insight into how fluoride interacts with enamel after application and why not all varnishes perform the same clinically.

Understanding What Happens After Fluoride Varnish Application

One of the most important takeaways from the article is that the not all fluoride uptake is equal. Once fluoride varnish is applied, it absorbs moisture from saliva and begins releasing fluoride ions. During this process, two different types of fluoride form on the teeth: loosely bound fluoride and enamel-bound fluoride.

Loosely bound fluoride forms very quickly on the surface of the teeth after application. While this form of fluoride can temporarily act as a fluoride reservoir, it is relatively soluble and can easily dissolve away. This form of fluoride provides little to no clinical benefit.

Enamel-bound fluoride, however, develops more gradually, over hours and days, as fluoride becomes incorporated directly into the tooth mineral. This process transforms hydroxyapatite into fluorapatite, a mineral that is significantly less soluble and more resistant to acid attack. Unlike loosely bound fluoride, enamel-bound fluoride can persist for months and is considered the primary source of durable enamel protection.

Why Contact Time Matters

A major focus of the article is the importance of varnish retention. Dr. Carey explains that enamel-bound fluoride takes time to form, meaning fluoride varnish must remain on the tooth surface long enough for this reaction to occur. Varnishes that quickly rinse away or disappear shortly after application may not allow optimal fluoride uptake.

The article highlights research showing that varnishes with adhesive carriers can produce significantly greater enamel-bound fluoride uptake than non-adhesive or water-based formulations. In some cases, adhesive varnishes demonstrated as much as three times the enamel-bound fluoride uptake compared to varnishes that washed away quickly.
